About The Initiative
Meet The Moment is a community-based educational initiative focusing on empowering African-American males aged 18-35. Our primary objective is to utilize a peer educator approach to educate and raise awareness among these young men about the risk factors associated with Chronic Kidney Disease (CKD). The program aims to equip them with knowledge on how to mitigate these risks and navigate the available treatment options for CKD through preventative and affirming messaging that encourages and reinforces a positive lifestyle change.
Meet The Moment
Chronic kidney disease (CKD) affects more than 1 in 7 U.S. adults—an estimated 37 million Americans.1 For Americans with diabetes or high blood pressure—the two most common causes of kidney disease—the risk for CKD is even greater. Nearly 1 in 3 people with diabetes and 1 in 5 people with high blood pressure have kidney disease.1 Other risk factors for developing kidney disease include heart disease and a family history of kidney failure.

Peer Educators and Ambassadors approach to CKD prevention and organ donation will be best served to reach the population of interest. Successfully training community-dwelling individuals aged 18-35 to serve as these educators and providing our target audience (community-dwelling individuals ages 18-35) with ongoing health and prevention messaging, health affirmations, and resource guides via Meet The Moment website and social media platforms will help participants understand their CKD risk, prevention messaging, affirming messages, and links to information will help encourage and reinforce positive lifestyle changes.
What We Believe
It establishes a sense of relatability and trust among the target audience, as peers from the same demographic can better understand and address their unique concerns and barriers to healthcare. This approach promotes open and honest conversations, enabling individuals to ask questions and share their experiences without fear of judgment. Moreover, peer educators can serve as living examples of successful preventive measures, inspiring and motivating their peers to adopt healthier lifestyles. By engaging with peers, individuals are more likely to absorb and act upon the information provided, leading to improved awareness and adherence to CKD prevention strategies, ultimately reducing the burden of CKD within the African-American male community.
